Dandruff

Dandruff is most commonly caused by seborrheic dermatitis, a chronic inflammatory condition of the scalp. It’s related to an overreaction to yeast (Malassezia) that normally lives on the skin, along with increased oil production and skin barrier sensitivity. Dandruff is not caused by poor hygiene and is not contagious.

Common symptoms include white or yellow flakes, scalp itching, redness, and irritation. In more severe cases, dandruff can extend beyond the scalp to the eyebrows, sides of the nose, ears, or behind the ears. Symptoms often fluctuate, with periods of improvement and flare-ups.

Ketoconazole 2% shampoo is a prescription antifungal treatment that reduces the yeast associated with dandruff and helps calm scalp inflammation. It’s one of the most effective treatments for controlling flaking and itching when used consistently. For best results, it’s usually left on the scalp for several minutes before rinsing.

Clobetasol 0.05% solution is a high-potency topical corticosteroid used short-term to reduce significant scalp inflammation, redness, and itching during more severe dandruff flares. It’s typically used for limited periods rather than continuously, to safely control symptoms while antifungal therapy does the maintenance work.

Many patients notice improvement within 1–2 weeks of starting treatment, especially with ketoconazole shampoo. For more inflamed flares, clobetasol solution can provide faster relief. Because dandruff is often chronic, ongoing maintenance therapy is usually needed to prevent recurrence.

Seborrheic dermatitis tends to be a long-term, relapsing condition. Even when symptoms improve, the underlying tendency remains. Stress, cold weather, illness, and changes in routine can all trigger flares. Maintenance treatment and gentle scalp care help keep symptoms under control.

Treatment frequency depends on severity. Ketoconazole shampoo is often used several times per week initially, then tapered to a maintenance schedule once symptoms improve. Clobetasol solution is usually reserved for short-term use during flares rather than daily long-term application.

Dandruff itself does not directly cause permanent hair loss. However, significant inflammation or frequent scratching can contribute to temporary shedding. Treating the inflammation and reducing itching helps protect scalp health and minimize shedding.

Heavy oils, pomades, fragranced products, and harsh scrubs can worsen dandruff by irritating the scalp or trapping yeast. A simple routine with gentle shampoos (alongside medicated treatments when prescribed) usually works best.

Often, yes. Teledermatology works well for classic dandruff and seborrheic dermatitis because diagnosis is usually based on appearance and symptoms. In-person evaluation may be recommended if symptoms are severe, spreading, not responding to treatment, or associated with significant hair loss.
